PER-model®

The PER model® provides Pedagogical, Emotional, and Relationship-building tools that allow the person with dementia to experience greater security in their existence and can stay in their home longer, while the relative receives support and relief.

The research results show that the method strengthens the sense of coherence, especially in the person with dementia, and contributes to a meaningful, comprehensible, and manageable everyday life.

The PER model in brief

  • Requirement for a doctor’s diagnosis.
  • Information about diagnosed individuals is sent to the support unit after consent.
  • An information brochure is distributed at the time of diagnosis with contact details to the support unit.
  • Support immediately after diagnosis with tailored information in groups or individually about the illness, daily life, and the municipality’s support and efforts.
  • Support Group 1: for individuals with more than 20 points on the MMSE-SR test (2) and their relatives.
  • Support Group 2: for individuals with fewer than 20 points on the MMSE-SR test and their relatives.
  • Support group for working relatives, such as adult children, meeting in the evening.
  • Relative Group: for individuals with close ones in day activities or in residential care.

Core values

The PER model® is based on a salutogenic perspective, focusing on health factors, meaning the health aspects of early-stage dementia patients and their relatives are supported and serve as the foundation of the work.

It supports participants towards greater manageability, comprehensibility, and meaningfulness in everyday life with the aid of education, emotional support, and relationships.

A salutogenic care and support system also liberates the abilities and resources of the disabled and their relatives.

Creators

The PER model® was developed by the following creators:

Annika Kjällman Alm: Licensed nurse, specialized in Elderly Care, researcher and Lecturer, Department of Health Sciences at Mid Sweden University.

Pär Hägglund: Head of the unit for relative and close support in Härnösand municipality. Holds a Master’s degree in Social Work with extensive experience and knowledge in support for relatives and dementia care.
